medications & allergy list
footwear
any referral letters
Please note
Patients who fail to show, cancel, or reschedule an appointment and have not contacted our office with at least 24 hours’ notice will be considered as a No-Show and charged a $50.00 fee which is not refundable through Medicare, and payable before booking the next appointment.
